      • Will replace figure 3.3.1 with a figure similar to the attached CDA Care Plan Instanciated with the skin integrity example while ensuring that at least one of the MCC conditions are incorporated.
      • Will edit the figure 3.2.2 FHIR profile based diagram to make it clear the MCC Conditions can be either a Health concern or a Condition (or both)
      • Will also take care to avoid the impression that a care plan is a "nursing only" process
      • will make corresponding text edits
      • Will either make corresponding changes in the same diagrams that are on the Care Plan profile page or consider housing them in one location and pointing to them from the other.

    Description

Figure 3.3.1 returns to adding Health Concern as the driver for interventions, many of which are listed in figure 3.2.2 but seemingly unrelated to a Health Concern or Condition.  It think this drawing (3.3.1) is trying to indicate a referenced Condition and its respective health concern and that two conditions reference the same health concern - but I don't understand that the outcome for the same health concern is different - is that intended to be the Goal (and therefore each has a different goal) or the actual outcome - and that means the patient has 2 PulseOx results two hours apart.  The representation is quite confusing.  And I think the actual Health Concern is missing from the diagram - I would consider that it is "impaired oxygen exchange" or something like that.  Moreover, the acronym RDS in the diagram is not defined anywhere so I can't be sure what is referenced.
