Description
I expect a Multiple Chronic Condition consideration to list Conditions, each with respective health concerns and overlap such that the plan addresses the Health Concerns to lead to better Condition-specific outcome. However, diagram 3.2.2 does not even include Health Concerns. There is no clear description of the 3.2.2 diagram to help a reader understand how these various elements fit together. There seems to be some magic between the MCC Chronic Disease Conditions, but I would expect the Health Concern to be the connection for all to allow related goals, interventions, and outcomes to roll up to the various Conditions as needed. That would also address the potential overlap of Health Concerns across the multiple conditions and address the potential conflict of goals that might be contraindicated by one or more of the related conditions. I don't see any discussion of these issues.
