Explain connection between Health Concern and Condition for MCC

      • Will replace figure 3.3.1 with a figure similar to the attached CDA Care Plan Instanciated (in FHIR-41910) with the skin integrity example while ensuring that at least one of the MCC conditions are incorporated.
      • Will edit the figure 3.2.2 FHIR profile based diagram to make it clear the MCC Conditions can be either a Health concern or a Condition (or both)
      • Will also take care to avoid the impression that a care plan is a "nursing only" process
      • will make corresponding text edits
      • Will either make corresponding changes in the same diagrams that are on the Care Plan profile page or consider housing them in one location and pointing to them from the other.

      When the first draft of diagrams and text changes are complete - we will review with the commenter and the PC WG Care Plan group

    Description

      Having some difficulty navigating (3) Structure and Design Considerations. The vision today seems to start with a Health Concern that causes establishment of a Goal which directs an intervention that leads to an outcome. That is relatively direct and understandable. However, there is no indication of a Condition in the diagram. I suspect any individual Condition (not yet multiple) may encompass multiple Health Concerns and that some Goals address 1 or more Health Concerns with respective interventions and outcomes.  That represents the "Vision Today".  Please explain.
