The consultant pharmacists in the Long Term and Post Acute Care (LTPAC) setting are required, by Center for Medicare and Medicaid Services (CMS), to exchange clinical recommendations based on a monthly drug regimen review (DRR) on every resident in the LTPAC setting. DRR documentation is a quality criteria for LTPAC Medication Management.

While specific to LTPAC, this consultant pharmacist consult note can be used by pharmacists in all their settings to exchange standard clinical data to the patient’s primary provider.

This project will create a joint HL7/NCPDP Consultant Pharmacist Consult Note guidance document. The guidance document will address the use of either C-CDA Consult Note or C-CDA on FHIR Consultation Note Profile. This will be a guidance document, not an implementation guide or FHIR profile.

Previously, NCPDP published the NCPDP Consultant Pharmacy Consult Note guidance document to address how C-CDA Consult Note is to be used to address Consultant Pharmacy reporting requirements..

This project will update the NCPDP Consultant Pharmacy Consult Note guidance document as described above to address both the C-CDA Consultation Note and the C-CDA on FHIR Consultation Note Profile. The product of this project will be published as a joint NCPDP/HL7 document.

Note: Initial review has not revealed any additional constraints or extensions on the FHIR Consultation Note Profile, thus allowing this project to produce a guidance document. If constraints or extensions become necessary, then a FHIR profile will be developed and jointly published, following HL7 processes to revise a project.
