Please document use cases for NDH Trust Framework extension

      Add description to the structure definition regarding use of the extensions and a note to describe potential uses of the trust framework extension

      Add guidance indicating that for trust frameworks that use private PKI there is no need to use this extension unless the goals is to provide access to endpoints via the endpoint reference on any of the relevant resources (e.g., careteam, healthcareService)

    Description

      There is next to no documentation in the IG for this extension, making it very difficult to evaluate its efficacy and appropriate usage. My remarks below are based on what I have been able to glean from discussions.

      Based on the above, my hunch is that this entire extension is not needed for endpoints in the national HIEs like the ones I mentioned. I'd like to see guidance that reflects that.
