Instead of creating the following extensions, place the identifiers as slices of Claim.identifier

gfeProviderAssignedIdentifier
interTransIdentifier
gfeServiceLinkingInfo.linkingIdentifier

There is no need for the Linking identifier to be located inside of the complex serviceLinkingInfo extension
