Add populationBasis element

The Quality Measure IG introduced a populationBasis extension as a way to identify the types of individuals in a population (this is currently inferred from the result type of the population criteria expressions). To make it explicit, the Measure should add a populationBasis element with a type of FHIRAllTypes to identify the expected type of individuals in the population.
