Explain whether maxValueSet prevents use of additional/translation codes

      Will update the wording to say "This acts as an overall 'required' binding for an element that already has a less restrictive binding.  It is used when there is a desire to have a 'small' enumerable binding that meets most needs and where the overall value set that the codes must be drawn from is 'infinite'.  The the base less restrictive binding SHALL be a proper subset of the max binding valueset.  In most cases, this extension is used where the base binding is 'extensible'."

    Description

      http://build.fhir.org/extension-elementdefinition-maxvalueset.html says this extension defines "the value set from which additional codes can be taken from." Can we please add an explanation of whether this constraint applies to all Codings present in a CodeableConcept? For example, if there is a preferred binding to a LOINC Vitals valueset, then a resource instance that includes a LOINC code from that set is fine, and including (in the instance) any additional translations or text is also fine. If you add a Max binding (to all LOINC) on the profile, does this make them instance with translations fail to validate? (If not, then what exactly is the intended validation logic?) The documentation should include an example like this and explain the result. (I'm genuinely unsure what the intention is.)
