The Advance Directives Section, Organizer, and Observation were taken from the Supplemental Guide.  There have been minor changes that were not deemed substantive and thus they kept their current template ID extensions. It is probably good practice for the Advance Directives guide to indicate that the proper versions of these templates are in the C-CDA guide now, but that is out of scope of this issue.
