Description
The current structure of the SRM and SIU message are ambiguous as currently written.

It is possible to have a PV2 segment without a PV1 segment. And it's not clear if the Observations and Diagnosis are associated with the Patient or the Visit.
A Patient Visit segment group should be added that at a minimum encloses PV1, PV2 and PRT.
