      Cardinality is correct

      #3 Update cardinality to 1..1 in "This value SHALL contain zero or one [0..1] @code (CONF:4536-143)" - because this entry is only expected, when there is a code based on this sentence: If it comes from a paper form, put the text label in the root Observation.code.originalText, not here
