Feedback on Observation vs. extensions for FHIR

    Description

      Using Observation resources to communicate SFCU, Gender Identity, Name to Use, and Pronouns should not be necessary.

       

      A common argument in favor of using Observation is the need to track provenance.  In some discussions, it is suggested that Provenance can only track resource-level provenance, not element-level.  However, there is a targetelement extension that can be used on Provenance.target that enables the Provenance resource to describe the specific element (or extension) within a resource to which it applies.

      My recommendation is to continue with the extension approach within FHIR, and in the uses cases where element-level provenance is necessary (if there are any such non-imaginary use cases), suggest the use of https://build.fhir.org/extension-targetelement.html.
