In the opioid examples the incorrect placement of the cqf-strengthOfRecommendation and cqf-qualityOfEvidence extensions outside of their defined context was causing the build to fail.  The extensions were moved directly under PlanDefinition.action.  The presence of the xsi:schemaLocation tag in the low suicide risk example also caused a validation issue, and it was removed.  The 'url' elements were also removed, as they are not needed for example instances and their presence with an HL7 url results in the "When HL7 is publishing a resource, the owning committee must be stated using the http://hl7.org/fhir/StructureDefinition/structuredefinition-wg extension" errors.
