As noted in the disposition for https://jira.hl7.org/browse/FHIR-43463 we will add an extension to improvementNotation to allow for free text entry of guidance related to the improvement notation so it does not need to be place in another field that is unrelated. This will also allow for guidance to be included to assist with risk adjusted measure calculations.
