change binding strength for DetectedIssue.severity

    Description

      The binding strength for DetectedIssue.severity to the https://hl7.org/fhir/6.0.0-ballot1/valueset-detectedissue-severity.html value set should be lowered from "required" to either "extensible" or "preferred".  This value set contains only 3 severity levels (low, moderate, high), but for some use cases for DetectedIssue, there are more than 3 severity levels.  Health systems often filter out results by severity level, so if there are in fact 5 severity levels, then it's important for all 5 levels to be represented in DetectedIssue.severity.  Otherwise, there will be too many or too few alerts.
