      There is already an ability to have an instance with content and no specified versions.  You only need to use the 'versions' structure if you have more than one.

      In the case where you have multiple versions, there is utility to have titles and descriptions for the overall instance, as well as for the different versions.

      For example:

      Instance 1: Title = "Initial prescription", Description = "Initial order for drug X"
        V1: Title = "Initial Draft", Description = "Unsigned draft of original order
        V2: Title = "Signed order", ...
        V3: Title = "Completed order", ...

      Instance 2: Title = "Renewal prescription", ...

       

      If we collapsed version and instance together, this would no longer be possible.

    Description

      Consider simplifying the structure so that version is at the same level as instance. If there is only a single instance with a particular key, there is only one version; if there are multiple instances with that key, each has to have a different version. Is there currently meaning to the instance-level data that is different from the version-level data?
