USCDI Health Assessment Value Sets are Missing Some Panel Codes

      This commenter is referring to the terminology table on the Screenings and Assessments page:

      The rows represent available "starter sets" of codes for the different USCDI Data Elements and the last three columns represent the type of codes (panel, panel item, or clinical judgement).

      A panel code should NOT be in the same set, as codes appropriate for the contained questions (aka "panel items") as the panel code would be at a "higher level", whether you are using the US Core Observation Screening Assessment Profile or US Core Observation Screening Assessment Profile for your Assessment Questionnaires.

      The exception to this rule is Social Determinants of Health Screening Assessments and Questions and Physical Activity which have both panel and item codes in it.

      This table needs to be completed, for example, there are USCDI Data Elements that only have panel code value sets and vice versa.

       

      Decision

      1) RE:

      The Exercise Vital Sign panel code is not included in the Physical Activity Observation Questions represent only panel item codes

       

      The commenter is correct, panel codes are not in the same set as codes appropriate for the contained questions (aka "panel items"). 

       

      2) RE:

      Substance Use value set contains codes for the Tobacco Panel items but not the Tobacco Panel code...

      this [Substance Use]value set is also marked as Alcohol Use despite having non-alcohol codes).

       

      The commenter is correct we are reusing the same valueset across USCDI Data Elements because they overlap.

       

      4) RE:

       

      The Alcohol Use Panel codes value set contains the AUDIT-C panel code, but not the AUDIT code.

      A panel code should NOT be in the same set as codes appropriate for the contained questions (aka "panel items").   The AUDIT-C panel codes are in the Alcohol Use Panel codes value set.

       

      To clarify the use of these valuesets rewrite this section:

      1) include a legend describing the rows and indicate whether they are clinical judgment, panel or panel item codes.

      2) note that in several instances a suitable value set has not yet been identified.

      3) add a note to Physical activity that Physical Activity Level codes can be used too.

      4) add Mental status Observation codes value set as clinical judgement codes

      5) Clarify that the Substance Use value set Includes Alcohol Use.

      6) correct that the Drug MIsuse codes are clinical judgment codes and not panel item codes?

      7) Align AUDIT panel and item codes in the appropriate value sets

    Description

      There are some panel codes missing from the value sets for Screening and Assessment codes:

      • The Exercise Vital Sign panel code is not included in the Physical Activity Observation Questions value set, though the two questions for the assessment are in the set. For the SDOH value set, the panel and question / item codes are both included. If an organization is distributing the Exercise Vital Sign similarly to SDOH screenings, they will likely want to structure the outgoing documents similarly, with the panel Observation and the question Observations marked with the same category. Including the panel code in the value set would help encourage sending the assessment questions as linked (via a derivedFrom Observation) instead of on their own.
      • Similarly, the Substance Use value set contains codes for the Tobacco Panel items but not the Tobacco Panel code (this value set is also marked as Alcohol Use despite having non-alcohol codes).
      • The Alcohol Use Panel codes value set contains the AUDIT-C panel code, but not the AUDIT code. The panel items value set has all of the AUDIT question codes, so the panel set should contain both panel codes.
