Don't imply that standardized queries are the norm

      The authors of this universal-realm guide are not comfortable indicating whether standardized or unstandardized queries are going to be more common for a given use case in a particular jurisdiction.

      We agree that we should not imply that standardized are more common but disagree that the guide should state unstandardized are more common either.

      Propose updating the IG to describe both with as unbiased a view as possible with a note that realm or use case specific guides can indicate preference.

    Description

      "Specifically, in cases where the data necessary is not well-standardized, it is unreasonable to expect the referring facility to be able to construct the queries necessary to retrieve the data. For example, in the United States, there is no standardized query to retrieve the current insurance coverage information for a patient."

       

      I use this example often, but I don't think it is US specific.  If you have Cerner and Epic in non-US countries, you'll have the same issue. And there are very few standardized queries in FHIR that I'm aware of.  US Core has a few for USCDI, and I think MedMij has a few too, but the large majority of the queries are not standardized in the FHIR world today.

       

      My suggestion for this paragraph is:

       

      "It is generally unreasonable to expect the referring facility to be able to construct the queries necessary to retrieve the data unless there is a use-case specific implementation guide with strict definitions of required queries for specific categories of data, or if there is direct pre-coordination regarding what queries to use between the parties involved."
