      Based on the related Tracker's FHIR-43355 disposition (see these slides for background Smoking Status.pdf

      1. Remove the max binding valueset from Observation.valueCodeableConcept
      2. Update the Observation.valueCodeableConcept binding to an updated extensible bind to an improved Tobacco Use value set, which will be shared with CCDA.
      3. (omitted from the  FHIR-43355 disposition)  Add Observation.valueQuantity as a Must Support type choice to support result values for Observation.code codes:
        1. 8664-5|Cigarettes smoked total (pack per year) - Reported
        2. 8663-7Cigarettes smoked current (pack per day) - Reported
      4. (omitted from the  FHIR-43355 disposition)  Add invariants
        1. For Observation.code 8664-5|Cigarettes smoked total (pack per year) - Reported  or 8663-7|Cigarettes smoked current (pack per day) - Reported SHOULD use valueQuantity

    Description

      The Smoking Status observation profile includes a binding to US Core Smoking Status Max-Binding ((https://hl7.org/fhir/us/core/2024Jan/ValueSet-us-core-observation-smokingstatus-max.html)).  When reviewing that, it just references all of SNOMED which does not add any value, plus creates confusion.  Suggest that this binding is removed and be fully aligned with C-CDA that does not reference the full SNOMED vocabulary for this.
