      The USCDI definition:

      Specimen Identifier
      Sequence of characters assigned by a laboratory for an individual specimen.

       

      The Base FHIR Specimen resource definitions:

      Specimen.identifier
      Element Id Specimen.identifier
      Definition Id for specimen.
      Short Display External Identifier
      Note This is a business identifier, not a resource identifier (see discussion)

    Description

      Considering that the USCDI definition addresses the accession number specifically, and would be the most common identifier to be associated with a specimen, and considering that FHIR provides a separate attributes specifically for accession number (Specimen.accessionIdentifier), we suggest that more appropriately the Specimen.accessionIdentifier is a Must Support and the Specimen.identifier a permissible field.
