      Using StructureDefinitions

      Providing refined definitions, comments/usage notes and examples for the elements defined in a Resource to reflect the usage of the element within the context of the Profile

       

      The current FHIR Core short  and description for Specimen.condition

       

      US Core version 7.0.0-ballot short, description and comments for Specimen.condition

       

      Rationale

      The FHIR guidance on profiling clearly permits "comments/usage notes and examples for the elements defined in a Resource to reflect the usage of the element within the context of the Profile".   The context of the US Core Specimen is the USCDI Data Class and, in this case, the USCDI Data Element, Specimen Condition Acceptability.    Our goal is a clear path from the USCDI Data Element to the US Core element.  The Specimen element's short definition is not in plain language that many of our readers will understand and may lead to confusion.  Additionally, the ONC data element is in plain language and provides additional context that is helpful to the reader when using the profile. Therefore, we 1) updated the short description and 2) added the ONC language to the Specimen.condition comment field to provide clarity.  Note that we did not update the element definition.

      Decision

      1) We will update the introduction and short description from "specimen condition acceptability". to "specimen condition"

      2) We will update the comment from:

      Information regarding a specimen, including the container, that does not meet a laboratory's criteria for acceptability. Examples include but are not limited to hemolyzed, clotted, container leaking, and missing patient name. This may include information about the contents of the container, the container, and the label.

      to:

      The specimen condition is used by the laboratory to determine the acceptability of a specimen for the ordered test. An individual specimen condition may be acceptable for one test, and not for another. The specimen condition is typically provided when the specimen does not meet the laboratory’s criteria for acceptability whether the test was performed or not.

      Examples include but are not limited to hemolyzed, clotted, container leaking, and missing patient name. This element may include information about the contents of the container, the container, and the label.

      3) To increase its visibility, we will move the comment from the Specimen.type comment field to a Profile Specific Implementation Guidance bullet, and keep the existing Specimen.condition comment from FHIR Core.

    Description

      We note that the short description and comments of Specimen.condition in the US Core profile ("Specimen condition acceptability") are substantially different than in the core Specimen resource's .condition definition and description ("State of the specimen", "A mode or state of being that describes the nature of the specimen." , "The specimen condition can be used to assess its quality or appropriateness for a specific test.", and "Specimen condition is an observation made about the specimen. It's a point-in-time assessment. It can be used to assess its quality or appropriateness for a specific test.").

      The condition is about the specimen itself, not about the suitability and acceptability for a particular test.  We suggest that US Core reflects in the attribute definition the correct meaning of Specimen.condition and describes in a comment (not short description or definition) that the specimen condition can be used (input) to assess the acceptability of the specimen for a particular test.
