Rather than saying what the human readable should look like, focus the language on what elements need to be displayed:
Local identifier, the code, the code system, and the display
And then make clear that the human readable is an example of a display that meets these criteria.

    Description

The human readable example that is being suggested more aligns with a QDM approach for showing direct-reference codes and not a FHIR one. In a FHIR measure, there is a section in human readable for direct-reference codes, refer to following:

      https://hl7.org/fhir/us/cqfmeasures/Measure-EXM55-FHIR.html

      https://hl7.org/fhir/us/cqfmeasures/Measure-EXM146-FHIR.html

This latter approach aligns with the current ballot: For reference:

      https://build.fhir.org/ig/HL7/cqf-measures/Measure-EXM55-FHIR.html

      https://build.fhir.org/ig/HL7/cqf-measures/Measure-EXM146-FHIR.html

So it seems that what is in Using FHIR with CQL ballot is incorrect
