Update definitions in the code system this value set is based on EP/ EC and EP/CAH to remove the term "CMS Promoting Interoperability" from the definition.

In addition, tracker https://jira.hl7.org/browse/FHIR-44642 will Add Quality Program value set as an example binding on useContext to publishable manifest profile in QM IG (derive from CRMI manifest). Note this work does not need to be done as part of resolving this tracker.

Remove the code "ep-ec" from the Quality Program value set as it is not available in FHIR because it isn't a MIPS program.
