It is probably a minor issue but the target code for a mapping should be either absolute or relative. This applies to :

insurance (ServiceRequest) --> .insurance OR ServiceRequest.insurance
extension:information-recipient.valueReference.identifier (Composition) --> .extension:information-recipient.valueReference.identifier OR Compostion.[element]
