      Clinician makes a simple observation - (for example, the patient states they are homeless)

      Clinician administers a survey to gather information (for example, the Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE) Survey)

      Clinician makes a “clinical judgment” based on an assessment tool. (for example, based on the PRAPARE survey, the patient has a lack of access to transportation)

       

      When there are multiple Assessments of a health-related matter like Disability Status, each Simple Observation is typically an independent Observation.  The survey option listed above allows for Questionnaire, QuestionnaireResponse and Observations using the  US Core Observation Screening Assessment Profile to link all the assessments together.  

      Other options for grouping multiple Observations are implementer specific decisions and guidance on grouping observations is provided in the FHIR specification.

      There may be cases where implementations have multiple observations at once for the same code at the same time. Disability Status is an example, where a clinician may note both vision and hearing impairment (observation.value) during a single point in time. Currently, the cardinality for Observation.value is 0..1. It would be ideal to allow multiple values to be represented within the same simple observation, similar to a lab panel that has multiple nested results without having to piece it together using the derivedFrom or hasMember field.  Would it be possible to get more examples in the spec on how to approach this use case, when a single simple observation using the same code can have multiple values.
