If DTR is not available for a given order/encounter and additional information is needed, there will NOT be a Questionnaire specified in the Coverage Information extension.  Payers SHOULD provide a 'link' card that indicates to the user where the relevant form(s) can be retrieved or a portal where information can be submitted.  The link SHOULD NOT require user authentication (i.e. no log-on needed) when accessing downloadable forms.  For portal links, it is preferred if a separate logon is not needed (e.g. with temporary/high-entropy links).  

Forms downloaded from provided links can then be submitted as part of the prior authorization (e.g. PAS), claim submission, etc. based on the identified documentation purpose.
