To benefit and payment status there is the option of in network, out of network, and other. To this, there should be the concept of a limited negotiated status for an out of network provider or service.

Negotiated:   A single, one-time rate that will be used to reimburse a service provided.   Negotiation has occurred due to an out-of-network provided service with a negotiated rate of reimbursement.
