Remove the following sentence: "Diagnoses are usually considered unfavorable, but they may also represent neutral or favorable conditions that affect a patient's plan of care (e.g., pregnancy)."
This sentence is not necessary for understanding the implementation guide. The sentence also assumes how a patient feels about any given diagnosis; a patient is best suited to determine how they feel about their diagnosis.
