"In the US, some practitioner Identifiers require that we specify the state, in addition to the identifier. We have no idea what the system will be - but need to indicate the state that issued it. For example, each DME provider in the US must have a state license for the state in which they will provide/sell DME. An ambulance that furnishes services in 1 state may drive over state lines to pick up and transport the patient to another state."
