Add Procedure.category in Course Summary because not inherited from mCODE anymore

      Procedure.category and ServiceRequest.category is bound to a valueset containing the new SNOMEDCT 1287742003 "Radiotherapy (procedure) and the inactive 
      108290001 "Radiation oncology AND/OR radiotherapy (procedure)" for backwards compatibility.   An invariant warns if the inactive code is used.  (Slicing with a preferred/max VS and was unsuccessful due to publisher limitations/definitions.  This invariant approach achieves the same result).

    Description

      mCODE removed the Procedure.category code from the Radiotherapy Course Summary in mCODE 3.0.0. CodeX RT still requires this code for all Radiotherapy ServiceRequests and Procedures to have a high level grouping into a common category. Therefore, CodeX RT should set the Procedure.category (category = SCT#108290001) in the Radiotherapy Course Summary Profile (as it does in all other ServiceRequest and Procedure profiles) to prepare for mCODE version from which this is not inherited anymore. The effective requirement for the Course Summary category will then be the same as with CodeX RT STU1 even if switching to a newer mCODE version.
