Import MedicationRequest from US Core 6.1.0 with the following additions/modifications:

Note that, unless the element has a MS flag in US Core 6.1.0, we should include those Keep element in the Key Elements Table for QICore but not add an MS Flag. Add (QM) to description of all Keep or Add elements such that that will be included in the Key Elements

The following table only lists changes to the import of US Core 6.1.0 elements to QI-Core 6.0.

      QI-Core Profile-based path Action Include in QI-Core 6.0 Justification
QICoreMedicationRequest.dispenseRequest. dispenseInterval Keep YES Not MS in US Core - used to calculate CMD
QICoreMedicationRequest.dispenseRequest. validityPeriod Remove NO Not MS in US Core - used to calculate CMD — Remove since meaning is how long the pharmacy may dispense the medication if the patient does not pick it up. This does not represent the period of time covered by the medication provided during each dispensing event.
QICoreMedicationRequest.dispenseRequest. expectedSupplyDuration Keep YES Not MS in US Core - used to calculate CMD
QICoreMedicationRequest.reported[x] Keep with modification YES (Change) Required element in US Core MedicationRequest and the item is a USCDI element. US Core includes reference only to USCorePractitioner. For QICore change reference to QICorePractitioner, , QICorePatient, QICoreRelatedPerson since the field is required in US Core to identify a patient-reported medication and have it appear on the medication list.  REMOVE current QI-Core reference to QICorePractitionerRole – it is an overreach since determining specific details about the practitioner making the request is likely to be challenging to collect and retrieve.
QICoreMedicationRequest.reasonCode Add YES USCDI element in US Core 6.0 with extensible binding to US Core Condition Codes.  NEW for QI-Core 6.0 (not present in QI-Core 5.0.1)
QICoreMedicationRequest.reasonReference Add YES USCDI element in US Core 6.0 with reference to base FHIR Condition and Observation.
NEW to QI-Core - Reference to QICore Condition Encounter DiagnosisQICore Condition Problems Health ConcernsQICore Observation
QICoreMedicationRequest.statusReason REMOVE NO  Not present in QI-Core 5.0.1 or in US Core 6.1.0 or USCDI - the element is referenced in QDM to QI-Core mapping tables as potential guidance but not as direct mapping and such mapping should be removed in QI-Core 6.0. (will be separate tracker)

QICoreMedicationRequest.doNotPerform Keep YES (modify from US Core) US Core Key Elements table even though not MS. For QI-Core, assure the profile is disjoint from QICoreMedicationNotRequested, QICoerMedicationRequest.doNotPerform has a Fixed Value: false.
QICoreMedicationRequest.subject Keep YES (modify from US Core) Required USCDI element in US Core and required to indicate the request is for a patient. Reference to QICorePatient rather than USCorePatient
QICoreMedicationRequest.encounter Keep YES (modify from US Core) Required USCDI element in US Core - reference QICoreEncounter instead of USCoreEncounter
QICoreMedicationRequest.requester Keep (Modify) YES (CHANGE) Required USCDI element in US Core. Reference QICorePractitioner instead of USCorePractitioner.
REMOVE reference to QICorePractitionerRole and QICorePatient since PractitionerRole data are unlikely to be retrievable and since US Core and patients cannot write medication orders so a patient-reported vaccine should be entered as ordered by practitioner (requester) and reported by patient.
QICoreMedicationRequest.recorder Remove NO US Core does not have MS or requirement for recorder. Requester should be sufficient
QICoreMedicationRequest.dosageInstruction.text Keep YES MS in US Core and needed for text prescriptions
QICoreMedicationRequest.dosageinstruction.timing Keep  YES MS in US Core and needed for cumulative medication duration
QICoreMedicationRequest.dosageinstruction.timing.repeat Keep YES Not in US Core MS but needed to evaluate cumulative medication duration
QICoreMedicationRequest.dosageInstruction.timing.frequencyfrequencyMaxperiodperiodMaxperiodUnit Keep YES Not in US Core MS but needed to evaluate cumulative medication duration
QICoreMedicationRequest.dosageInstruction.asNeeded[x] Keep YES Not in US Core MS but needed to evaluate cumulative medication duration
QICoreMedicationRequest.site Remove NO Not currently used and not MS in US Core
QICoreMedicationRequest.route Remove NO Not currently used and not MS in US Core
QICoreMedicationRequest.method Remove NO Not currently used and not MS in US Core
QICoreMedicationRequest.doseAndRate Keep YES MS in US Core and USCDI element
QICoreMedicationRequest.dose[x] (doseQuantity) Keep YES MS in US Core and USCDI element
QICoreMedicationRequest.courseOfTherapyType Do Not Add No While this element could add value to identify continuous use medications that are prescribed 'as needed' (PRN), the element remains available from the Snapshot Table if desired. While some organizations use this element as a choice box on PRN orders it has not yet been used in existing measures.
