The US Core Laboratory Result Observation profile is a subset of Clinical Result. Propose updating the QI-Core 6.0 to import US Core 6.1.0 as is with the following modifications based on QI-Core 5.0.1 (I.e., REMOVE indicates remove from QI-Core, ADD indicates add to QI-Core either due to presence in US Core or eCQM requirements):

      QI-Core Profile-based Path Action Include in QI-Core 6.0 Justification
Observation.status Keep YES USCDI – Import US Core Laboratory Result Observation Profile and modify value set binding to required Non-Negative Observation Status. This is a subset of the required US Core binding value set removing the cancelled option to allow a disjoint with QICoreObservationCancelled.
QICoreObservation.category Keep YES USCDI – Import US Core Laboratory Result Observation Profile; slices for category with preferred binding to ObservationCategoryCodes; category:us-core with required binding to US Core Clinical Result Observation Category and coding complex with code Fixed Value = laboratory
QICoreObservation.code Keep YES (Modify Value set binding) USCDI – Import US Core Laboratory Result Observation Profile and maintain extensible binding to US Core Laboratory Test Codes.  This represents a change in the binding currently used in QI-CoreLaboratoryResult.
QICoreObservation.subject Keep YES (modify reference) USCDI – Modify reference from US Core Laboratory Result Observation Profile to refer to QI-CorePatient
QICoreObservation.[effective[x] http://hl7.org/fhir/us/qicore/STU5/StructureDefinition-qicore-observation-lab-definitions.html#key_Observation.effective[x]] Keep YES USCDI – Same as US Core effectiveDateTime using datatype dateTime
QICoreObservation.[value[x] http://hl7.org/fhir/us/qicore/STU5/StructureDefinition-qicore-observation-lab-definitions.html#key_Observation.value[x]] Keep YES USCDI – not US Core notation SHOULD use SNOMED CT for coded results.
Also, US Core limited values to Quantity, Codeable Concept, ValueString.  Continue to include valueBoolean, valueInteger, valueRange, valueRatio, valueTime, valueDateTime, valuePeriod
QICoreObservation.dataAbsentReason Keep YES USCDI – US Core extensible binding to DataAbsentReason
QICoreObservation.interpretation Keep YES Not included in US Core/USCDI but has value similar to lab tests as discovered by eCQM community. Retain extensible binding to ObservationInterpretationCodes
QICoreObservation.specimen ADD YES USCDI – included in US Core referencing US Core Specimen Profile – specimen used for this observation. US Core 6.0 is the first occurrence of the Specimen profile. Reference the QICoreSpecimen profile
QICoreObservation.component REMOVE NO Not used in existing measures. Rather than requesting nested laboratory tests, eCQMs have used specific LOINC codes to indicate the desired lab test.
QICoreObservation.component.code REMOVE NO Not used in existing measures. Rather than requesting nested laboratory tests, eCQMs have used specific LOINC codes to indicate the desired lab test.
QICoreObservation.component.[value[x] http://hl7.org/fhir/us/qicore/STU5/StructureDefinition-qicore-observation-lab-definitions.html#key_Observation.component.value[x]] REMOVE NO Not used in existing measures. Rather than requesting nested laboratory tests, eCQMs have used specific LOINC codes to indicate the desired lab test.
QICoreObservation.component.interpretation REMOVE NO Not used in existing measures. Rather than requesting nested laboratory tests, eCQMs have used specific LOINC codes to indicate the desired lab test.
QICoreObservation.issued ADD YES Not in US Core or QI-Core 5.0. At least one hospital harm measure evaluated time from availability of a result (hemoglobin result) to action (transfusion) rather than to the physiologic time of the test (specimen time). Therefore, the issued, availability time is important to include.
QICoreObservationClinicalTestResult.basedOn DO NOT ADD NO This element comes from the QDM to QI-Core mapping. Any relationship can be managed using CQL timing expressions.
QICoreObservationClinicalTestResult.partOf DO NOT ADD NO This element comes from the QDM to QI-Core mapping. Any relationship can be managed using CQL timing expressions.
