Add Sex Extension that aligns with CCDA Sex Template Observation as a USCDI Requirement

Keep Birth Sex Extension in US Core, but it is no longer a USCDI Requirement. In other words, it is an optional extension.

Update examples, USCDI mapping tables, versions changes, etc to reflect these changes.
