Claim and ClaimResponse can be used for claim submission, or for prior auth.  In the case of claim submission, you are submitting the claim for an encounter, so having a .encounter reference to Encounter makes sense.

However, for prior auth, you don't have an encounter yet, so you'd need to support a referenced to the Appointment that the prior auth is associated with.
