Add text to the definition and the description:

Used in STU3 and earlier terminology services. The information it provides should be communicated with operation outcome code = too-costly by a terminology server based on a more recent FHIR version.

The extension http://hl7.org/fhir/StructureDefinition/valueset-toocostly needs better documentation - when is it ok for a server to return this? What obligations does it create?

Note that I am doing more work around this, and I may propose to withdraw this (deprecate) and only use valueset-unclosed
