Need to distinguish device-created from transcribed observations

    Description

      When the Device is the sole performer (i.e. the device is making the measurement and is the sole asserter of its truth - common for auto measurements of heart rate, step counts, etc.), and thus there's an Observation.device but no Observation.performer, there's a need to reflect whether the data in the Observation instance flowed directly from the device into the record electronically, or whether the data was transcribed.  Obviously we could capture transcriber with Provenance, but the absence of a transcriber wouldn't be enough to safely infer electronic flow.

      Open mHealth and Agonaut 'write' both proposed an extension for this.  Suggest that we define a common core extension called "capture" with definition "Indicates how this information flowed from the device that reported the value shown in the record".  Comment "Only relevant if the observation was gathered from a device, though it's possible that details of the device used won't be present."  Extensible value set:

      "electronic" - data from the device flowed electronically into the record with no manual intervention (though transformation may have occurred)

      "transcribed" - data from the device was displayed, printed or otherwise rendered for human consumption and subsequently entered into the record, typically through manual intervention.
