Improve measure narrative based on Connectathon feedback:

1. Add population descriptions to metadata table
2. Add population criteria grouping for primary population criteria
3. Get example that has values for all elements so the example illustrates all possible values
4. Add descriptions to the logicDefinition
5. Make sure usageCOntext is part of the narrative
6. Only display the display portion of codes in the human readable
