This text "For example, the Medication Reconciliation Post Discharge measure example included in this implementation guide references the Healthcare Effectiveness Data and Information Set (HEDIS) Implementation Guide, which defines profiles specific to that particular HEDIS measure." should link to the HEDIS IG.  If that IG is no longer available (which seems to be the case?  My google search came up empty.), then the reference should be removed, or it should at least be noted that the HEDIS IG is no longer available.
