The statement below in Section 3.6.2.1 is misleading. It fails to instruct users about the relevance and use cases for using Observations when gathering information which may contribute to demographic information often carried within a FHIR Patient resource.
Not all questions and answers in an assessment tool may or should be represented as FHIR Observations. For example, the FHIR patient resource best represents patient demographic information.
There are valid reasons why information about a patient's race or ethnicity, language spoken, or gender may be gathered from a certain source using a particular method. When the source and method information for those observations is relevant, then it may and should be gathered using an Observation.
The above italicized paragraph should be modified to provide a balanced and more instructive statement that clarifies why some patient demographic information may, should, or in some cases, shall need to be gathered first using observation resources which include the source and method information which indicates the provenance of the demographic information.
"Not all questions and answers in an assessment tool may or should be represented as FHIR Observations. However, there are many use cases when Observations should be used. For example, although the FHIR Patient resource represents patient demographic information, it often is important to initial record the demographic information using Observations which record the source and method used to collect the demographic data."
