Updating the eCare Clinical Plan As changes are made to the targets of the eCarePlan references, the MCC CarePlan will include those references through the link. As interventions are performed, the reference targets from the eCarePlan are updated. For example, if the CareTeam gains a team member, the CareTeam instance referenced by the MCC CarePlan will reflect that change. Other updates to the eCare plan, such as adding new interventions, are achieved by adding additional references in the appropriate data element.
