    Description

      1. Multiple Chronic Condition Care Plan Caregiver Considerations Observation profile allows multiple performers in the observation (Observation.Performer card is 0..*)
      2. Observation.focus allows a required single reference to Multiple Chronic Condition Care Plan Caregiver on Care Team (1..1)
      3. If there are multiple caregivers in the performer element that needs caregiver considerations will result in a careTeam resource for each performer. Suggest profiling this in a way where the Observation.focus element can reference the relatedPerson directly rather than a relatedPerson in a care team resource. The reason for doing so is because in the source system, the observation needed for the caregiver consideration will be on the performer and can be used directly.
