      The current guidance for scopes is optional:

      > Where appropriate, the EHR MAY expose these controls using SMART scopes as follows.

      When required, EHRs can lock scopes down via code, as in the example at http://hl7.org/fhir/smart-app-launch/2023Jan/app-state.html#provider-facing-companion-app-in-the-ehr

      Together, these points should allow implementers to gain experience with this new experimental capability; based on real-world experience we can revisit enhancements to the scope language.

    Description

      The various example scopes in section 5.7 do not distinguish between giving an app launch access to the main FHIR server or to a different app-state server, or between access to an app-state Basic resource vs other Basic resources.

      Clarify if this is intended, and if not, devise app-state scopes.
