Will be removing Multiple Chronic Condition Care Plan Caregiver on Care Team Profile and so no need to add slicing, but will be making the following related changes:

Retain Multiple Chronic Condition Care Plan Caregiver Considerations Observation (may need to modify name) . Edit Profile text as appropriate

Remove Care Team from observation.focus and change to RelatedPerson

Observation.value will hold the "issue"
