      1. The “Relationship Between Screening and Assessment Observations” header in section 3.6.2.1 creates the impression that the diagram below might be about the relationship between “screening observations” and “simple (clinical) observations”. Rather, it is about the relationship between a screening questionnaire/survey and the screening observations generated from each question (whether single select or multiselect). Consider renaming to “Relationship Between Screening Assessment Observations”.
      2. The profile name on the left states “US Core Observation Screening and Assessments Profile”. That should state “US Core Observation Screening Assessment Profile”
