In section 3.6.1 there is a section called “Screening and Assessment Activities” with a diagram that indicates a cycle about “Screening and Assessment Activities” (indicated by the label in the center circle. However, the only item in the workflow that addresses “screening and Assessment” is the top circle. All of the other circles are other activities. What this diagram is really indicating is something like the “Assessment and Care Cycle”, “Assessment and Care Activities”, or “Assessment, planning, intervention, and evaluation”. Please update the title and diagram to reflect the fuller scope of the diagram.
