The description in section 3.6.1 states “US Core Simple Observation Profile - a clinician’s decision to document a specific item - “patient has problems with housing”. Note this observation is only appropriate when the judgment isn’t appropriate on the list of Problems or Health Concerns.”
This is not the description of a simple observation, nor does it encapsulate the scope of the use of this profile for assessments properly. See (FHIR-39940) regarding the naming of this profile. If the name is appropriate (because the scope is greater than just assessments), then note that simple assessments are captured using the simple observation profile. Otherwise, please rename the profile.
The description states that this is “a clinician’s decision”, however, the profile supports more and there is no restriction to allow other types of performers, such as a patient or related person. A simple assessment should allow for more than just a clinician’s decision, so please update the description to be something like “an assertion or determination (such as a clinician’s observation)…”
The description should also indicate that this profile is to be used when the assessment is something other than a screening response (to help delineate it’s use from the screening assessment). (Screening derived assessments (responses to some sort of question in a tool) should not use this profile)
