Suggest to remove the slices and bind Coverage.class.type to group and plan

The current slice rendering is clearer.  Minimal benefit to this change also mimics the Carin BB and Da Vinci Guides design.

There is no real difference between the group and plan slice

Based on feedback from payers, group and plan are important distinctions.  These distinctions are also modeled in Carin BB and DaVinci HRex.

    Description

Coverage.class is sliced, yet there is no real difference between the group and plan slice. Suggest to remove the slices and bind Coverage.class.type to group and plan, while keeping that extensible. It seems that having to create slices to document and achieve hit sis overly complicated.
