In FHIR, we generally try to NOT use terminology-based relationships because this totally messes up the 80%.  We would much rather have explicit relationships (and a bunch of explicit extensions) rather than a name-value pair that mixes relationships that are widely used with relationships that are rarely supported, or in some cases, never supported.  Please revert this change
