Why is the "care team claim scope" needed?

      The careTeamScope extension is used for intermediaries so they know in which segments to put the care team member information.

      Regardless of that, the Claim.item has a careTeamSequence to point to a care team member involved in that specific claim item.  It is possible for an item to point to a care team that is claim scope and not specific to an item.  It is also possible for an item to point to a care item member involved in a different item.

      The careTeamSequence and the careTeamScope are really orthogonal data points.

      We noted that the detail Claim.item has a subelement called "care team sequence".  This appears to be redundant.  

      If the intent is that you don't use the Claim.item.CareTeamSequence when the CareTeam: Overall Claim Member slice is present, it would be useful to give that guidance at the Claim.item.careTeamSequence element.
