For item 2, forenameInitials is not the same as middleInitials. Middle initials can already be represented using the `given` element of the HumanName datatype. Forename initials are not specifically handled within HumanName.

In the Citation resource, please clarify whether forenameInitials is the same as middle initials. If not, please add an element for middle initials.
